Should i get help? I dont know what to do. I'm scared of living, of going on. I always struggle with life. But every few months it gets really bad and i end up in the psych ward. The question is either: Going to the psych ward voluntarily or trying to commit suicide and getting admitted in the closed off one. I'm so tired of life. It helped me going there once because i was completely suicidal. The other time they didnt even take me, they told me pretty much to just stop drinking alcohol, stop doing drugs and getting my shit together. I'm scared and i dont want to do. I live in a shared apartment with caretakers and if this goes on much longer i'll get admitted either way probably. So go on risk and try committing suicide or go on the other risk and try getting help?

I dont know....if i tell them i'm suicidal its pretty much sure that they'll admit me, if i want it or not. Thats pretty much the key word. Here in austria theres a lot you can say to get around all of it. But if you ever say you want to kill yourself its over and youre in there faster than you can say: "fuck!"

If you explore counseling options outside of established systems and institutions, you are pretty much limited to 12 step groups or small churches.

The way I see it, option 2 does not take away option 1. I am in therapy, and if it is not enough to make me a person who can live, it does not keep me from killing myself. I am free to leave therapy at any time, wether it is to stop therapy and to die. Do you need help finding somewhere to start?

I do have therapy and I'm in a good system. The getting help part was more about going into the psych ward

Have you tried anti depressants,? If there is any chance of you being helped to cope with life, take it. What harm can it do to seek help, not necessarily for suicidal ideation, but depression. Much love to you.

I'd guess @timf might live in the US, where there are indeed limited options, and some people do go the 12-step route due to not being able to afford therapy. [I don't know whether it is a large number of people who do that, but I know that it does happen.]

Also, the OP did mention substance usage, so a 12-step program might be a good thing to pursue (in addition to the other types of treatment). Giving up alcohol and drugs can be very helpful to recovery from mental illness, but I think the person has to actually WANT to do that (and pretty badly) for 12-step programs to "work." However, there are also some 12-step programs for things other than substances that might be a good fit and helpful, if a group can be found.

It sounds like you're struggling a lot with your mental health and substance use issues, so first I want to validate how difficult all of this must be.
Second, I recommend that you consider attending an intensive outpatient program. (Also called "day hospital /day program, or Partial Hospitalization Program.)
These types of programs are a step in between "normal" (weekly) therapy and psych ward. You would attend the program for 3-5 hours per day during the weekdays, and then go home to sleep, watch TV or do whatever you want. Typically, you spend most of your time doing group therapy, but you also get individual sessions and a doctor to manage your psych meds.

I have done those programs before, and personally, I highly recommend them. It's a great way to feel like you're getting a lot of help, but you are not completely giving up your freedom.
